THE FOLLOWING CONVENIENCE FEE WILL APPLY TO YOUR TOTAL CHARGES:
$ AMOUNT FEE CHARGE
$1 – $50 = $3.00
$51 – $150 = $6.00
$151 – $250 = $9.00
$251 – $350 = $12.00
$351 – $450 = $15.00
$451 – $550 = $18.00
$551 – $750 = $21.00
$751 – $950 = $24.00
PLEASE NOTE: FEE AMOUNT WILL BE ADDED TO ACTUAL CHARGE CARD AMOUNT WHEN PROCESSING.

If you need to report a public health emergency or Class A reportable disease after hours, please contact the Van Wert County Sheriff’s Department at 419-238-3866 and they will contact the health department personnel on call. If you are experiencing a medical emergency, please dial 911. If you need to report a public health risk, problem, environmental hazard, or reportable infectious disease, please call 419-238-0808 and follow the prompts to leave a message for the appropriate person.
