The Van Wert County General Health District and other community agencies formed a partnership called the Van Wert County Health Collaborative. The purpose of the formation of the collaborative was to collect data to show the health and health behaviors of Van Wert County residents and identify the health needs. The data collected was then analyzed and provided in the Community Health Needs Assessment (CHNA). The CHNA serves as the foundation for development of a Community Health Improvement Plan (CHIP) to guide Van Wert County for the next three years. The CHNA will be assessed and priorities determined by the collaborative, who will then work together to focus on addressing these priorities.
The progress relating to our local priorities will be monitored with measurable indicators identified by the Van Wert County Health Collaborative. The individuals that are working on action steps will meet on an as needed basis. The committee will form a plan to disseminate the Community Health Improvement Plan to the community. Action steps, responsible person/agency, and timelines will be reviewed at the end of each year by the committee. Edits and revisions will be made accordingly.
